INFLUENCE OF THE INFLAMMATORY IMMUNE RESPONSE IN PATIENTS WITH CUTANEOUS LEISHMANIASIS AND POSITIVE SEROLOGY FOR TRYPANOSOMA CRUZI.
Introdução
In several regions of Latin America, the geographical distribution of cutaneous leishmaniasis (CL) overlaps with areas of transmission of Chagas disease. Studies show the rate of Leishmania sp. / T. cruzi ranging from 12 to 70%. The skin lesion caused by L. braziliensis is associated with an exacerbated inflammatory response, where high levels of TNF, IL-1 and Granzyme B are associated with the severity of the CL.
Objetivo (s)
The aim of this study was to determine the prevalence of coinfection L. braziliensis and T. Cruzi in an area of transmission of L. braziliensis , and evaluate the immune response of these patients.
Material e Métodos
A case-control study in which patients with CL caused by L. braziliensis, living in the endemic area of Corte de Pedra, Bahia, were selected. Sera from one hundred and eighty patients with cutaneous leishmaniasis were evaluated with specific chimeric proteins from Trypanosoma cruzi to detect coinfection with Chagas disease using the ELISA technique.
Resultados e Conclusão
The prevalence of coinfection L. braziliensis and T. cruzi was 11%. Coinfected patients had higher serum IL-6 levels than individuals with CL alone. There was no difference in serum levels of IFN-γ, TNF, IL-1β between coinfected patients and patients with CL alone. Regarding the clinical outcome, therapeutic failure to pentavalent antimonial was observed in 70% of coinfected patients, while 42% failed treatment in the group with LC only. These results show that coinfection L. braziliensis and T. Cruzi may increase the inflammatory response of these patients and the time for cure of leishmaniasis..
