NEGATIVE LEISHMANIA SKIN TEST PREDICTS POOR CLINICAL AND THERAPEUTIC OUTCOMES IN CUTANEOUS LEISHMANIASIS: ROLE OF GRANZYME B AND PARASITE BURDEN
Objetivo (s)
This study aims to compare the clinical, immunological, and pathological differences between LST-positive and LST-negative patients with cutaneous leishmaniasis caused by L. braziliensis (CL).
Material e Métodos
Comparison of clinical aspects and therapeutic outcome of 134 CL patients with positive (67) to negative (67) LST, as well as histopathology of 40 of those CL patients (20 in each group) and cytokine levels of 20 of those patients (10 in each group).
Resultados e Conclusão
The study found that LST-negative patients had a significantly larger and greater number of lesions and a longer duration of illness compared to LST-positive patients. Additionally, therapeutic failure of meglumine antimonate was significantly higher in LST-negative patients (23 in LST-positive vs. 45 in LST-negative). The study also found that granulomas were seen much less frequently in LST-negative patients (11 in LST-positive vs. 3 in LST-negative). Additionally, LST-negative individuals had considerably higher levels of free granzyme B (882.1 pg/ml in LST-positive vs. 1408 pg/ml in LST-negative) and less IFN-γ (160.5 pg/ml in LST-positive vs. 30.6 pg/ml in LST-negative).
In conclusion, LST-negative CL patients have more parasites, a worse treatment outcome, less IFN-γ, and a higher expression of granzyme B in the lesion tissue compared to LST-positive CL patients.
