Título

Genomics and epidemiological data reveal arbovirus transmission dynamics in Rio de Janeiro, Brazil.

Introdução

Arboviruses are endemic in Rio de Janeiro State, presenting challenges in clinical diagnosis due to symptom similarities with other diseases. This study aims to enhance public care services in the state of Rio de Janeiro by investigating the transmission dynamics of circulating arboviruses in the different state's mesoregions.

Objetivo (s)

We sought to understand the spatial and temporal dynamics of the main circulating arboviruses (ZIKV, CHIKV, DENV) in the state of Rio de Janeiro, with the aim to identify the potential determinants of their dispersion and associated clinical outcomes.

Material e Métodos

Arbovirus data including number of notified and confirmed cases were obtained from the Brazilian National System of Notifiable Diseases (SINAN). For convenience, cities were aggregated into mesoregions (n=6). Case counts and epidemiological curves were obtained using the R language and available whole genome sequences from those viral pathogens were submitted to phylogenetic reconstruction.

Resultados e Conclusão

Epidemiological data revealed seasonal epidemic peaks in each mesoregion and suggested that the Metropolitan mesoregion reported the highest number of arbovirus cases, with peaks of dengue and chikungunya occurring between 2018 and 2020, and Zika cases concentrated in 2016. Phylogenetic reconstructions revealed a complex transmission dynamic across the state with multiple introduction events occurred troughs time. Enhanced epidemiological surveillance of arboviruses is necessary to control, mitigate and prevent potential epidemics and gain a deeper understanding of their dispersion in the state.
